# Transition Readiness

## When

-   Planning a move to a less restrictive setting.
-   IEP and transition meetings.

## Why

-   Rate of acquisition and retention come from first-trial probes the tools already record.
-   You still choose every score; the data is there to support it.

## How

1.  Open **Transition Readiness** with the learner selected and tap **New assessment**.
2.  Read the data under each area, then tap a score from 1 to 5.

**Saved to the dashboard:** Your score for each area, per assessment date, with the latest total and rater agreement on the dashboard.
